SF Health Surveys with PIQ-6

Designed for efficiency, the SF-36v2® Health Survey with PIQ-6™ and the SF-12v2® Health Survey with PIQ-6™ simultaneously measure overall health, pain severity, and the impact of pain on functional health and well-being.

At the core of each survey is QualityMetric’s Pain Impact Questionnaire™ (PIQ-6™), in addition to either the SF-36v2® Health Survey or SF-12v2® Health Survey. While the SF-36v2 and SF-12v2 measure overall health, the PIQ-6 measures the severity of pain and its impact on those suffering from mild to severe pain symptoms, within a variety of diseases and general populations.

Both surveys are available in U.S. English with a standard four-week recall period, and can be administered online or via fax capability.
