Child Health Questionnaire (PF-50, PF-28)
The Child Health Questionnaire™ (CHQ) PF-50 is a widely used generic measure of a child’s functional health and well-being. In addition to providing a profile of a wide range of domain scores, the CHQ™ PF-50 also has the capability of estimating physical and psychosocial health summary measures. The domain and summary scores have been shown to be reliable, valid, and responsive to change across a wide variety of conditions and treatments.

This survey is intended for children between the ages of 5 and 18, and is available in multiple language translations with a standard four-week recall period.
