Child Health Questionnaire (PF-50, PF-28)

The Child Health Questionnaire™ (CHQ) PF-50 is a widely used generic measure of a child’s functional health and well-being. In addition to providing a profile of a wide range of domain scores, the CHQ™ PF-50 also has the capability of estimating physical and psychosocial health summary measures. The domain and summary scores have been shown to be reliable, valid, and responsive to change across a wide variety of conditions and treatments.

The CHQ PF-50 is a precise measure that is supported by numerous peer-reviewed publications and is an ideal tool for applications including clinical trial studies, population health monitoring, and burden of disease analysis. This survey is intended for children between the ages of 5 and 18, and is available in multiple language translations with a standard four-week recall period.

The Child Health Questionnaire™ (CHQ) PF-28 is an alternative short-form to the CHQ PF-50. The CHQ™ PF-28 is ideal when a shorter measure of child health status is needed, and is available in multiple language translations with a standard four-week recall period.
