Integrating QualityMetric's SF-12v2® Health Survey into your disease management, case management, or wellness program is a simple, cost-conscious way to evaluate its effectiveness and performance.
Here's how it works: Program participants are asked to complete the two to three minute survey as they begin the program. The survey results are automatically scored via our Smart Measurement System and available to the participant and program administrator in real time.
At regular intervals chosen by the administrator, participants retake the survey. Scores are recorded and progress is tracked over time. This allows each participant to see his or her own progress. It also allows the program administrator to track both individual performance and overall program results.
Because the SF-12v2 is scientifically valid and offers comparisons to population norms in addition to individual results, it provides the administrator with a solid basis for evaluating the program and making intelligent decisions.
