John E. Ware, Jr., PhD – Chief Executive Officer and Chairman of the Board

Dr. Ware co-founded QualityMetric Incorporated in 1997, after serving for 12 years as Senior
Scientist at The Health Institute, New England Medical Center (NEMC) in Boston, and Principal
Investigator for the Medical Outcomes Study (MOS). From his work on the MOS, he created the SF-36®
Health Survey and other tools widely used to monitor patients' outcomes. Before moving to NEMC, Dr. Ware
spent 13 years as a Senior Scientist at the RAND Corporation. An internationally recognized expert in health
status assessment, John is a Research Professor in the Department of Medicine at Tufts University School of Medicine,
an Adjunct Professor in the School of Public Health at Harvard University, and a member of the Institute of Medicine,
National Academy of Sciences.

Charles Beckman – Director, Corporate Development and Venture Capital, Humana, Inc.

Mr. Beckman is responsible for the oversight of Humana's strategic investments, as well as being a participant in the
strategic planning and M&A function of the company. With 10 years in the health care industry, Mr. Beckman brings a diverse
background to the board. In previous positions with Humana, he was responsible for development and management of provider networks,
and for strategic planning for various Humana specialty products, including dental, group life, disability and workers compensation
services. Recently Mr. Beckman has played an integral role in the overhaul of Humana's long-term, planning and M&A function.

Allyson Ross Davies, PhD, MPH – Principal, ARD Consulting

Dr. Davies has established a pre-eminent consulting firm following a highly successful career in healthcare,
most recently on the leadership team at MassPRO, Inc., a health care quality improvement organization that provides
quality oversight for Medicare and Medicaid programs in Massachusetts. She spent the preceding two years on the
executive team at QualityMetric, where she led both business development and product and service development activities.
A nationally recognized expert in the measurement and use of patient–reported outcomes, she spent the preceding 13 years
consulting to and working in hospitals, managed care companies, and health systems on outcomes measurement and monitoring
systems. Dr. Davies spent another 12 years in health services and quality of care research at the RAND Corporation and UCLA.
She is the first non-physician chair of the board of the MassPRO, sits on the board of directors of the American Health Quality
Association, and is a founding director of QualityMetric.

James E. Dewey, PhD – Co-Founder and Past President, QualityMetric Incorporated

During the past 16 years, Dr. Dewey has provided key health care expertise in the development of software to
measure health risk and functional status and well–being. Before co-founding QualityMetric Incorporated in 1997,
Dr. Dewey served as Vice President of the Managed Care Division at HCIA Inc., and founder and Chief Executive Officer
of Response Technologies, Inc., which developed the RT–2000 Immediate Response Terminal and other point-of-care hardware
and software, including the first software to support collection, scoring, and reporting of SF-36 data.

Albert Waxman, PhD – Co-founder and Senior Managing Member, Psilos Group Managers, LLC

Dr. Waxman focuses on Psilos' e–health and health care services investments, identifying new investment opportunities. He
also serves as a board member, advisor, and liaison to the health care community for several portfolio companies. Dr. Waxman, nationally
recognized as one of the innovators in care management and health care services, plays an active role in defining Psilos' new media and B2B
e-commerce strategies.
