used to measuring physical functioning and mental well-being across eight (8) domains: physical functioning, role limitations due to physical health
(role-physical), bodily pain, general health perceptions, vitality, social functioning, role limitations due to emotional problems (role-emotional)

Reporting
Collected data, captured via QualityMetric's portfolio of SF™ Generic Health Surveys, offers real-time patient and population
reporting. This data is used to expand the understanding and interpretation of PROs across program sites, demographics and time.
This exclusive, customizable reporting structure provides flexibility in individual- and/or aggregate-level
reporting for patient and/or administrative and clinical use.
Individual-Level Patient Reports
The individual-level report allows users to view their specific health status data, as collected from one of the SF™ Generic Health
Surveys that have been administered. The report, at the individual-patient level, reflects how well an individual is managing his/her health.
Aggregate-Level Patient Reports
Aggregate-level reports allow for the interpretation of group PRO data to substantiate meaningful healthcare and utilization decision-making.
The aggregate-level report function enables clients to generate group health summary scores with the following filter criteria:
- Age
- Gender
- Date range
- Program names/sites
- Member status
Medical Expenditure Prediction
The Medical Expenditure reporting module enhances the interpretation of scores from the SF-36™ and SF-12™ Health Surveys based on
newly developed, scientifically valid, algorithms. SF Expenditure Prediction Solution provides a per-patient, per-month prediction
of medical expenditures along with age, gender and disease specific benchmarks that assist providers in identifying members/patients
at risk of increased medical costs using data collected from either the SF-36™ or the SF-12™ Health Surveys.
Outcomes-Insight™ Support Option
When a Quick Start PRO Assessment Solution package is purchased with the Outcomes-Insight™ Support Option, two (2) hours of
consulting services will be included in the purchase price to assist clients in any or all of the following program elements:
- Development of study designs and implementation
- Program and process validation and documentation
- Data interpretation/analysis
Additional Outcomes-Insight Support hours are available upon request at an additional cost.
