Background

  Recently, the Medical Outcomes Trust (MOT), the Health Assessment Lab and QualityMetric Incorporated - the co-copyright and trademark holders of the SF-36®, SF-12® and SF-8™ Health Surveys - merged their licensing and user registration programs. We have two objectives:

  • To simplify the licensing and user registration processes; and
  • To better meet the needs of the many new academic, commercial, and other users of these and the other survey forms, scoring algorithms, and norms that we offer.

Because of the rapidly expanding worldwide demand for commercial products and services, we are expanding our licensing programs for re-sellers and for commercially sponsored research that use our trademarks, copyrighted forms, scoring algorithms, norms, and other interpretation guidelines ("intellectual property") in the United States and in more than 40 other countries. We are also committed to making our tools and algorithms available to academic researchers worldwide.

Our overall goals for these programs remain unchanged:

  • We strive to maintain the scientific standards for our surveys and scoring algorithms that make results directly comparable and interpretable.
  • We remain committed to offering academic consideration to those individuals and organizations using our survey for un-funded scholarly research.

Our new shared licensing programs will assure that those individuals and organizations that benefit from commercial uses of the intellectual property pay royalties or other user fees that will support the research community that made the original surveys possible.

  Please complete and submit the online Information Request. There is no fee or obligation for doing so. The information you provide will help us determine the license or user registration program that best fits your planned uses of the intellectual property.

  • The Information Request asks you to indicate which survey(s) you or your organization plan to use. As price varies based on a variety of factors, we ask that you please submit our online Information Request to ensure you receive an accurate price quote.
