The Hepatitis Quality of Life Questionnaire™ (HQLQ) is a two-part survey that includes:
- The component summary measures
- A 15-item supplemental battery focusing on other general health concepts relevant to assessing the impact of
hepatitis (such as health distress and positive well-being) as well as indicators of hepatitis-specific distress
and of limitations in functional status related to hepatitis
The HQLQ was designed to help patients and clinicians monitor effects of the disease and its treatment on the individual's
functional status and well being. Useful both for screening and for monitoring changes in disease impact, the HQLQ is available
in a paper-and-pencil format that can be administered in clinical settings, at home, or in other locations.
The HQLQ can also be administered as a mail-back form or during telephone or face-to-face interviews.
This fixed-length survey can also be customized for online administration.
