Assessment Lab, co-copyright and trademark holders of the SF-36®, SF-12®
and SF-8™ Health Surveys, have merged licensing and user registration programs to:
- Simplify the licensing and user registration process
- Better meet the needs of academic, commercial, government, and other users of these and the
other survey forms, scoring algorithms, and norms offered
Our overall goals include:
- Maintaining the scientific standards for our surveys and scoring algorithms that make results
directly comparable and interpretable
- Remaining committed to an academic program for individuals and organizations for un-funded
scholarly research.
Our new shared licensing programs will assure that those individuals and organizations
that benefit from commercial uses of the intellectual property pay royalties or other user fees that
will support the research community that made the original surveys possible.
