The CHQ™ PF-50 is a widely-used generic measure the health status of children. The form can be completed by parents/caregivers of children ages 5-18. The survey
provides coverage of a wide range of domains (see table 1). In addition to a profile of the domain scores, the user has the option of estimating physical and psychosocial
health summary measures. The CHQ™ PF-50 scales and summaries have been shown to be reliable, valid and responsive to change across a wide variety of conditions and
