QualityMetric SF™ Generic Health Surveys capture reliable and scientifically valid patient-reported health
outcomes information and have been adapted for use in more than 80 languages. Used successfully in more than 1,000
randomized clinical trials reported in over 240 scientific and medical journals, these surveys are proven
responsive in 64 disease conditions and are accepted by the Food and Drug Administration as proof of benefit for
improved functioning and other patient-reported outcomes (PROs).
All QualityMetric generic surveys are available for paper/pencil administration; some are also available for
administration via faxing/scanning, telephone using voice recognition, PDA or online, with features that
include accurate data capture, scoring, and interpretation. Select QualityMetric surveys are also available
as faster, smarter computerized adaptive applications using our proprietary DYNHA® software.
While the SF-36® and SF-12® Health Surveys established the gold standard in PRO measurement, new
versions of these surveys developed by QualityMetric, the SF-36v2™ and SF-12v2™ Health Surveys,
have been shown to produce substantial improvements. The SF-36v2™ and the SF-12v2™ Health Surveys have been
adopted as the standard of measurement by key government agencies, including the Agency for Healthcare
Research and Quality, which includes the SF-12v2™ Health Survey in the National Medical Expenditure Panel Survey.
Generic Health Surveys
-
SF-36v2™ Health Survey
Based on the original SF-36 Health Survey, the SF-36v2 is used to compare health outcomes among groups of patients, to screen
individuals for a variety of health concerns, and to monitor health outcomes over time.
SF-36® Health Survey
-
SF-12v2™ Health Survey
As a brief, reliable measure of overall health status, the SF-12v2 is the instrument of choice in population health surveys
and has also been used extensively as a screening tool. Because of its brevity, it is frequently embedded in longer,
condition-specific surveys.
SF-12® Health Survey
-
SF-10™ Health Survey for Children
Developed to address the need for scientifically valid health status assessment for the pediatric population, QualityMetric's
SF-10 Health Survey for Children measures the health status of children ages five and over.
-
SF-8™ Health Survey
The SF-8™ Health Survey represents a major advance in the development of health outcomes assessments that are comprehensive yet extremely brief,
and works best in very large population surveys or other studies involving large samples and group-level comparisons.
-
DYNHA® Computer Adaptive Health Assessments
The SF-36v2™ Health Survey is also available through a faster and smarter computerized adaptive application
(DYNHA®) to achieve the most practical and precise measures used to monitor and compare generic health outcomes
across diverse populations.
