Information Collection and Use

QualityMetric Incorporated is the sole owner of the information collected on this site. We do not sell, share, or rent this information to others in ways different from what is disclosed in this statement. We do not accept any advertising on our website.

QualityMetric Incorporated collects other information from you or your computer at several different points throughout our website.

Demo (Demonstration) Survey

A user may choose to complete a demo survey. No information from demo surveys is collected, saved or stored.

Cookies

A cookie is a piece of data stored by a website on the consumer's hard drive, which contains information about how the consumer used the website. Cookies enable us to track and target the interests of our consumers to enhance the experience on our site. Usage of a cookie is linked to personally identifiable information while on our site. If a user rejects the cookie, they may still use our site. The only drawback is that the user will be limited in some areas of our site.

Log Files

We use Nettracker software that uses IP addresses, browser type, ISP, referring/exit pages, platform type, date/time stamp, and number of clicks to analyze trends, administer the site, track user movement, and gather broad demographic information for aggregate use. IP addresses, etc., are not linked to personally identifiable information. We do not sell or share your IP addresses, etc.

Choice/Opt-out

We provide you the opportunity to "opt-out" of having your personally identifiable information used for certain purposes when we ask for this information.

If you no longer wish to receive our eNewsletter and promotional communications, you may opt-out of receiving them by following the instructions included in each eNewsletter or communication, or by emailing us at webmaster@qualitymetric.com.

You will be notified when your personal information is collected by any third party that is not our agent/service provider, so you can make an informed choice as to whether or not to share your information with that party.

License Agreements

When you request a single or multiple user non-commercial license for one of our products, we ask you to complete a Permission to Use and/or a Survey Information Request Form. A user provides contact information including name, phone number, shipping address, email address, etc. We use this information to contact you if we have questions about your request or need additional information to process your request. We ask detailed information about your project to verify that you have chosen the correct license type and most appropriate survey for your project. We ask you additional questions about the purpose of your project, number of surveys to be administered, intervention being measured, publication intent, time frame, data collection, measurement type, and outcomes. This information is used to verify the type of license requested, verify that there is no commercial intent, and to determine a royalty fee schedule (if any). We do not sell or share any of this requested information.

SF-36® Partners Company Profile

When you request a commercial license, we ask you to complete the Partners Company Profile. A user must provide contact information including name, phone number, shipping address, email address, etc. We use this information to contact you if we have questions about your request or need additional information to process your request. We ask questions about your business, such as number of employees, annual revenues, and product information. We request this information to create your contract, calculate your annual fee, and determine your royalty fee schedule. We do not sell or share any of this information.

Sharing

We do not share any personally identifiable information, either individual or aggregate data, from this website. This company does not retain, share, store, or use personally identifiable information for any secondary purposes.

Security

We take every precaution to protect the information you have shared with us. When users submit sensitive information via our website, your information is protected both online and offline. When our Survey Information Request Form asks you to enter sensitive information, that information is encrypted and is protected with the best encryption software in the industry – SSL. While on a secure page, the lock icon on the bottom of web browsers such as Netscape Navigator and Microsoft Internet Explorer becomes locked, as opposed to unlocked, or open, when you are just 'surfing.' While we use SSL encryption to protect sensitive information online, we also do everything in our power to protect user information offline. All of our users' information, not just the sensitive information mentioned above, is restricted in our offices. None of our employees require consumer-specific information to perform their jobs. They only deal in the aggregate or grouped data.

For extra security, our employees use password protected screen-savers when they leave their desktop computer. ALL employees are kept up-to-date on our security and privacy practices. Annually, as well as any time new policies are added, our employees are notified and/or reminded about the importance we place on privacy, and what they can do to ensure our customers' information is protected.

Correction/Updating Information

If a user's personally identifiable information changes (such as your name or zip code), or if a user no longer desires our services, we will correct, update, and/or remove that user's personal data at their request. Notification of Changes
