The SF-12v2® Health Survey–MH Enhanced is the standard SF-12v2® Health Survey with the addition of five items from the Mental Health (MH) scale of the SF-36v2® Health Survey. This mental health PRO survey has been shown to be an effective screener for major affective disorders, such as depression, anxiety and loss of control.
Because the SF-12v2-MH Health Survey uses T scores, the health domain and component summary measure scores can be compared with those from other SF™ health surveys. For scoring interpretation, consult the SF-36v2® and SF-12v2® scoring range guide.

The SF-12v2-MH Enhanced Health Survey was developed using content from the SF-12v2® and the SF-36v2® Health Surveys. The MH version takes five key mental health items from the 36v2 and adds to the sf 12v2, offering additional insight into the patient’s mental health status.
